CDC: BRFSS -- Prevalence & Trends Data: The Behavioral Risk Factor Surveillance System (BRFSS) collects state data about US residents regarding their health-related risk behaviors, chronic health conditions, health-care access, and use of preventive services.

CDC: National Center for Health Statistics: Compiles statistical information to guide actions and policies to improve the health of the population. 

CDC: National Health Interview Survey - Interactive Data Query Systems: Provides annual estimates for selected characteristic of the U.S. population by demographic subgroup

County Health Rankings & Roadmaps: The annual data release provides a revealing snapshot of how health is influenced by where we live, learn, work, and play. The snapshots provide communities a starting point to investigate where to make change.

Oregon Health Authority - Data and Statistics: Collects and analyzes data on health behaviors, diseases and injuries, disseminates findings, and designs and promotes evidence-based programs and policies to improve the health and safety of all Oregonians.

Oregon Health Teens -- Health Data by County (2022)  

US Census - Quick Facts: QuickFacts provides statistics for all states and counties, and for cities and towns with a population of 5,000 or more
